This is where your care begins when you would like to speak with Dr. Badillo before completing laboratory testing.
This appointment is best for you if:
- You are a new patient
- You are unsure which laboratory tests you need
- You want to discuss symptoms, concerns, or goals first
- You want physician guidance before deciding on next steps

Review of your medical history and current concerns

What should I complete before the appointment?
Complete the BLL Initial Questionnaire and required consent forms before your visit. These are available through the patient portal and help Dr. Badillo prepare for your consultation.

Yes. In some cases, Dr. Badillo may recommend laboratory testing before the initial consultation so the first physician conversation can include your results.
Unless the practice has already recommended a labs-first approach, the Initial Physician Consultation is the best place to begin when you are unsure.
Is this included with membership?
The Initial Physician Consultation and written Personalized Plan may be included in your membership. Existing members should confirm their available benefits before submitting payment.
Blood Work
Concierge Phlebotomy
A mobile blood draw performed at your home, workplace, or another approved location after your labs have been ordered.
Schedule this service only after Badillo Longevity Lab has ordered your laboratory testing or instructed you to begin with blood work.
This appointment is best for you if:
- Your labs have already been ordered
- The practice recommended that you complete blood work first
- You prefer a mobile blood draw instead of visiting a lab center
- You are located within the available service area
What is included
Blood collection at an approved location
Preparation and coordination of laboratory specimens
Biometric analysis during the mobile visit
Coordination with Quest or LabCorp when appropriate
Where is concierge phlebotomy available?
Concierge phlebotomy is available in Chicago, Chicagoland, and select locations in Indiana.
Additional travel fees may apply for locations more than 25 miles outside downtown Chicago.
Can I go to Quest or LabCorp instead?
Yes. Concierge phlebotomy is optional. You may schedule directly with Quest Diagnostics or LabCorp after your requisition has been prepared.
Share your scheduled draw date and time with Badillo Longevity Lab so the requisition can be dated and sent correctly. Biometric analysis is only included when the draw is performed directly through Badillo Longevity Lab.
Do I need to fast?
Many laboratory panels require fasting for approximately 12 hours. Morning appointments are generally recommended.
Follow the specific instructions provided with your laboratory order because requirements vary by test.
How are laboratory charges and insurance handled?
Laboratory testing completed through Quest Diagnostics or LabCorp may be billed to your insurance. Coverage and out-of-pocket responsibility depend on your individual plan.
Update your insurance information in the patient portal before your requisition is prepared when you intend to use insurance. Coverage is not guaranteed and should be confirmed directly with your insurer.
Are there additional fees?
A $49 Lab Order and Handling fee applies to the preparation and curation of the laboratory panel. This fee is waived for members.
Charges from Quest Diagnostics or LabCorp are separate from the concierge phlebotomy appointment fee. Additional travel fees may also apply based on location.
Is concierge phlebotomy included with membership?
Concierge phlebotomy may be included based on membership tier:
- Foundation: 1 draw per year
- Personalized: 2 draws per year
- Individualized: 3 draws per year
- Full Concierge: 4 draws per year
Members should confirm their remaining annual benefits before submitting payment.

Schedule this appointment after your laboratory results have been completed and released to Badillo Longevity Lab.
This appointment is best for you if:
- Your laboratory results are complete
- The practice has confirmed that your results are available
- You want to review your biomarkers with Dr. Badillo
- You began with the labs-first path

Can this be my first appointment with Dr. Badillo?
Yes. Patients who begin with laboratory testing may meet Dr. Badillo for the first time during the Lab Analysis & Review.
New patients must complete the Initial Questionnaire and required consent forms before the appointment.
What should I complete beforehand?
Confirm that your laboratory results have been released to the practice. New patients should also complete all intake and consent forms through the patient portal.

Existing Patient
Follow-Up Visit
An ongoing care appointment for established patients who need to review progress and fine-tune their current plan of care.
This appointment is best for you if:
- You are an existing patient
- You have already completed your initial consultation or lab review
- You need to review your progress with Dr. Badillo
- You need adjustments to your current plan of care

Should I book a follow-up or a lab review?
Book a Lab Analysis & Review when you have a new set of completed laboratory results that require a detailed physician interpretation.
Book a Follow-Up Visit when you are checking in on an existing plan, discussing progress, or making ongoing adjustments.
Is this included with membership?
Follow-up visits may be included depending on your membership tier and remaining physician visits. Review your membership benefits before submitting payment.
Before You Schedule
Important booking information
These policies apply across appointment types unless otherwise noted.
Payment, deposit, and card requirements
Televisits and concierge appointments require a valid card on file and a deposit to reserve the appointment.
Credit cards, debit cards, HSA cards, and FSA cards are accepted. Card payments are subject to a 3% processing fee.
Insurance and superbills
Badillo Longevity Lab does not directly accept insurance for physician appointments. A superbill may be provided for possible out-of-network reimbursement.
Laboratory testing through Quest Diagnostics or LabCorp may be billed to insurance. Coverage and patient responsibility vary by plan and should be verified directly with your insurer.
Cancellation policy
Appointments canceled with less than 24 hours’ notice may be subject to a cancellation fee unless there is a valid emergency or another approved circumstance.
Membership benefits
Physician visits, written Personalized Plans, concierge phlebotomy, and certain additional services may be included depending on your membership tier.
